Above the volume, select your current playback device to display a list of playback devices available. Make sure you have the correct audio device selected. Right-click the Speakers icon. Select Speaker setup. Select the correct output channels for your device. If you are using standard, non-surround sound, select Stereo.

The real issue you were having is that Bethesda sends dialogue through the center speaker when using surround sound, which you are either lacking or there is a genuine bug. Hello, I have had a major issue for a while that NPCs just stare at me and no nothing, know dialogue, nothing. Sometimes main quests work, and sometimes companion works.
